Programs
C.W. WILLIAMS COMMUNITY HEALTH CENTER provides comprehensive services to meet the unique needs of its patient population. In addition to the comprehensive primary care services, C.W. Williams CHC offers the following special programs:
Diabetes Program
The purpose of this program is to provide a comprehensive approach to managing Diabetes. In addition to primary care, the program provides patients with ophthalmolic and nutrition services. C.W. Williams CHC is a member of the State Diabetes Collaborative and Bureau of Primary Health Care Diabetes Collaborative, which significantly helps improve the care of their diabetes patients.
HIV/AIDS Program
The purpose of this program is to provide primary care and specialty care services to HIV/AIDS patients and to offer adherence counseling and case management to address the total needs of these individuals.
Homeless Health Care Initiative
The purpose of the project is to provide primary care, nursing services, and
pharmaceuticals to the residents of Uptown Shelter and participants of Urban Ministry Center.
Mammography Screening Program
This program is a joint collaboration between C.W. Williams CHC, Mecklenburg County Health Department, and Our Lady of Guadalupe Church (a large Hispanic/Latino church). The purpose of the program is to provide breast health screening and education to the women of Metrolina and Our Lady of Guadalupe Church, targeting the Hispanic population. A bilingual health educator teaches breast cancer prevention and early detection classes.
Asthma Program - Project On TRAC
K.I.C.K.S. – Kids Increasing Confidence and Keeping in Shape
This program is sponsored by the Community Care Partners of Greater Mecklenburg and is designed to assist families who have overweight children. The goal is to raise awareness of this crisis and to promote healthy habits that can last a lifetime. This program provides education about healthy dietary choices and encourages more physical activity for children.
